about us

We care deeply about health and wellness, engage expert practitioners and pursue actions that enable people and organizations to thrive.

our vision

At Wellconnex, we envision a world where providers deliver proactive, integrated care that creates value for the healthcare system, while improving the health of the communities they serve.

For the Wellconnex team, integrated care occurs both within clinical teams and across an organization or healthcare system.

• At the clinical level, true integrated care looks like a team of diverse providers — from physicians to nurses to social workers to community-based navigators—collaborating to proactively care for the full needs of patients, from hospital to home.

• At the organizational level, integrated care strategies equip leaders and executives with the real-time data they need to align resource planning and contracting strategy to best support their clinical teams and patients while ensuring financial stewardship.

our mission

Wellconnex’s mission is to equip healthcare systems and their clinical teams with tools and strategies that promote proactive, integrated care. Our strategies and tools have been shown to simplify patient management and resource planning, optimize capacity, and improve clinical and financial outcomes.

We are an experienced consulting agency with proven solutions designed from the bottom-up to enable business leaders and clinical care teams to more efficiently and effectively manage large volumes of patients across their clinical programs and empower healthcare systems to deliver the best care possible for complex patients. By helping to more nimbly identify and engage patients that most need attention, our goal is to improve the quality of care and reduce unnecessary utilization, such as emergency room visits and hospital admissions.

the wellconnex story

Pressure to manage growing populations of patients in a cost-effective way has never been greater for healthcare systems and providers. For healthcare system executives, insurance and reimbursement reform have led to a wave of acquisitions fueled by an uncertainty about future revenue. This consolidation has created more complex healthcare systems with greater numbers of patients than ever before—increasing provider burnout and patient frustration over the difficulty they face accessing needed services in a timely way. These challenges have also become more acute following the COVID-19 epidemic, during which time, health systems have had to quickly pivot to telemedicine and other methods of proactively caring for patients remotely. While the emergence of the electronic health record has promised fast and actionable data at providers’ fingertips to identify and support patients most in need of services, there has been no cost-effective solution on the market that optimizes the electronic health record practically for this purpose.

Given these issues, when the founders of Wellconnex developed the model at Children’s Hospital of Philadelphia, the path forward for their population health program was uncertain. The team spent 5 years designing, building and refining solutions leveraging the power of the electronic health record to support the greater vision of proactive, integrated care they were seeking. With a peer-reviewed evaluation revealing a significant reduction in admissions and bed-days with a return on investment of $6 for every $1 invested in the program and evidence of improved patient outcomes, the model came into focus. Given that type of impact, it was clear that these solutions were ready to be exported to other health systems.

At Wellconnex, we believe delivering better patient care begins with deploying well-supported integrated teams—including physicians, nurses, social workers and community-based navigators—to more nimbly identify and engage patients that need the most attention. Therefore, Wellconnex offers a suite of solutions and services designed for and actively used by front-line clinicians and business leaders focused on rapid analytics, strategy development and evaluation, and proactive care management. These solutions are built within the electronic health record and provider workflows to simplify use and maximize adoption. Designed with leading disease experts, our tools and strategies are proven to promote integrated care, help direct more efficient decisions for the use of telemedicine, simplify patient management and resource planning, and improve clinical and financial outcomes.
